The patient portal supports your ability to request an appointment, refill prescriptions, review statements, update demographics, and add or update pharmacy information.
Need a Patient Portal Username and Password?
You will be given a user name and password at the time you schedule an appointment. If you did not receive a username and password when scheduling, please call (787) 764-8880 and we will provide one.
Forget your Patient Portal Username or Password?
Please call our office at (787) 764-8880 during normal office hours and ask for our staff to reset your user name and password. Given that your password is secure and known only to you, we cannot tell you what your password is, but we can provide a temporary reset password that will allow you to log-in and set a new secure password.
